What is included in homecare therapy?
At an individual’s home, our Physiotherapists and Occupational Therapists Sydney conduct an assessment or therapy session that typically lasts for 1-2 hours.
We use both subjective and objective assessments to determine the person’s current ability and create a clinically appropriate rehabilitation plan.
We also monitor progress towards goals and track any declines while providing recommendations for beneficial assistive equipment. Our team takes a holistic approach, inquiring about other areas of life, such as continence and vision management.
For homecare cases, we evaluate falls risk and conduct environmental reviews to promote safety.
To lower the risk of falls, we may:
- build lower limb strength
- prescribe bathroom rails
- prescribe shower chairs
- address fatigue components
Our clinicians provide rehabilitation strategies that focus on building strength, prescribing home exercise programs, and teaching safe strategies.
We also maintain detailed records of the session to accurately track progress.
Real Success Stories From Our Homecare Clients
Vista Healthcare was fortunate to help a lovely client that lived on a very big property.
The husband has advanced Parkinson’s disease and the wife had moderate to advanced dementia. Upon meeting these people, the wife was chair bound and struggled to stand without the assistance of a lifter or carer. Because of this, she spent a lot of time sitting in front of the TV.
The lady loved cooking and standing at her sink which overlooked the mountains and her lovely garden. It was her meaningful goal to develop the capacity to walk herself to the kitchen and safely cook dinner, look at the beautiful view and enjoy herself at home.
We worked together to safely practice walking. At first, it was just a few steps at a time, then a few more. Having a neurological condition, it is important to lower the risk of falling. Falling mostly occurs because someone cannot react fast enough to move their feet. So, we practice a lot of balance and sit-to-stand exercise together.
Vista Healthcare worked with the lady for several months, and in the end, the lady was able to walk to the kitchen and enjoy her beautiful home and independence. For the times where she was tired, she had access to a wheelchair to help her.
Home Care Package Client FAQ
- What does Vista Healthcare do in terms of scheduling, documentation and compliance?
In most cases, we will receive a client from a homecare provider, it is also possible for homecare clients to self-refer.
We will then perform an intake of the referral made, which is us typically phoning the referee and asking more details about the Physiotherapy/OT requirements and synergizing with the case coordinator.
We will organize our initial assessment at the persons residence to conduct an initial assessment. Vista Healthcare takes care of all scheduling. So, we will visit the client and communicate with them in the way they would prefer.
We store all clinical documentation using our practice management systems which can be provided to the homecare coordinator for their compliance, providing we are consented to do so. We value privacy and confidentiality.
In the current environment, it is important to know that we take the risks associated with COVID-19 seriously and all our staff have untaken the Department of Health COVID- 19 infection control online training for health workers in a community setting. This training covers the fundamentals of infection prevention and control for COVID-19.
